Analysis of the Value Chain of Tuberculosis Health Services at Medan Johor Health Center in 2018

Introduction

Tuberculosis (TB) is a significant public health concern in many parts of the world, including Indonesia. In 2017, Medan Johor Health Center recorded a striking number of TB cases, ranking second highest in Medan with 1,025 suspected cases. This study aims to analyze the quality of TB health services at Medan Johor Health Center from the perspective of patients, TB officers, and the Medan City Health Office. The study focuses on the value chain of TB health services, which is divided into two main categories: Service Providing Services and Service Support Activities.

Background

Medan Johor Health Center is a primary healthcare facility that provides essential health services to the community. In 2017, the center recorded a significant number of TB cases, with 539 (52.7%) cases in Pangkalan Mashyur Village, 290 (28.3%) in Kwala Bekala Village, and 196 (19%) in the Johor Gedung Village. This study aims to identify the strengths and weaknesses of the TB health services provided by Medan Johor Health Center and to provide recommendations for improvement.

Methodology

This study used an in-depth interview and observation method to collect data from 11 informants relating to TB problems at Medan Johor Health Center. The informants included patients, TB officers, and health officials from the Medan City Health Office.

Tuberculosis Health Services Quality

The results of this study showed that each variable analyzed produced a positive assessment from the informants, indicating that the quality of TB health services at Medan Johor Health Center was at a fairly good level. However, this research also identifies several deficiencies that need to be overcome, especially in terms of human resources and physical conditions of the Puskesmas facilities.

The existence of adequate human resources is very crucial in supporting the quality of health services. In the context of Medan Johor Health Center, TB analyst deficiencies can inhibit the patient handling process, ranging from initial detection to effective treatment. In addition, the physical condition of the Puskesmas that is less than optimal also needs to be considered. Adequate facilities and a clean and comfortable environment are very influential on patient satisfaction and health.
