Analysis of Differences in the Status of the Accreditation of the Performance of the Essential Public Health Efforts (UKM) at the Medan City Health Center

Introduction

The Community Health Center (Puskesmas) plays a crucial role in the implementation of Essential Community Health Efforts (UKM). However, the achievement of performance in environmental health programs, nutrition programs, as well as disease prevention and control programs (P2P) at Medan City Health Center is not optimal. This refers to the Puskesmas performance appraisal guidelines that show a shortage in the implementation of Essential SMEs. The results of the initial survey revealed that the management of the Puskesmas has not been able to implement the Essential UKM to the maximum, due to the lack of competence of health workers in accordance with the type of SMEs that must be carried out.

The purpose of this study is to identify differences in accreditation status related to the performance of essential SMEs, including health promotion, environmental health, KIA-KB, nutrition programs, and P2P at Medan City Health Center. The method used in this study is mixed-method, which combines a qualitative and quantitative approach. By using a sequential explanatory strategy, the population studied consists of all puskesmas in Medan, of which 39 of them are used as samples. Data analysis is carried out through univariate and bivariate analysis.

Methodology

The mixed-method approach used in this study combines both qualitative and quantitative methods to collect and analyze data. The qualitative approach involves in-depth interviews with health workers and community leaders to gain a deeper understanding of the challenges faced by Puskesmas in implementing Essential SMEs. On the other hand, the quantitative approach involves the use of statistical analysis to identify patterns and relationships between variables.

The population studied consists of all puskesmas in Medan, of which 39 of them are used as samples. The sample size was determined based on the availability of resources and the feasibility of data collection. The data collection process involved the use of questionnaires, interviews, and observations to gather data on the performance of Essential SMEs.

Results

The results of the study showed a significant relationship between the Puskesmas accreditation status and the Environmental Health Program (P = 0.027; 95% CI = 0.894-19,001), Nutrition Program (P = 0.048; 95% CI = 0.633-20.956), and P2P Program (P = 0.022; 95% CI = 0.697-10,465). Meanwhile, health promotion and KIA-KB do not show a significant relationship with the status of the Puskesmas accreditation.

The results of the study suggest that the Puskesmas accreditation status is closely related to the performance of Essential SMEs, particularly in the areas of environmental health, nutrition, and disease prevention and control. The findings of this study have important implications for Puskesmas management in Medan City.

With these strategic steps, it is hoped that the accreditation status of the Puskesmas in Medan can increase, so that the performance of essential SMEs becomes more optimal and has a positive impact on overall public health. Increased accreditation will also be an indicator of success in the implementation of sustainable health programs.
