The Influence of Individual Characteristics and Organizational Factors on the Performance of Village Midwives in ANC Services in the Work Area of the Puskesmas Babul Makmur District, Southeast Aceh Regency

Introduction

The provision of Antenatal Care (ANC) services is a crucial aspect of maternal and child health, particularly in rural areas where access to healthcare facilities is limited. Village midwives play a vital role in providing ANC services, but their performance is often hampered by various factors. This study aims to investigate the influence of individual characteristics and organizational factors on the performance of village midwives in ANC services in the work area of the Puskesmas Babul Makmur District, Southeast Aceh Regency.

Background

The coverage of ANC services in the working area of the Babul Makmur District Health Center in 2013 was unsatisfactory, with a K1 coverage of 81.1% and a K4 coverage of 68.0%. This reflects the suboptimal performance of village midwives in providing ANC services. The study aims to analyze various factors that influence the performance of village midwives, including individual characteristics such as knowledge, working period, residence, and training, as well as organizational factors, namely perceptions of leadership and supervision.

Methodology

This study was conducted between May and July 2014 using the Explanatory Survey Method. The entire village midwife population totaling 31 people at the Babul Makmur District Health Center was taken as a sample. Data was collected through interviews and observations using a questionnaire, and then analyzed using multiple regression analysis at the level of significance α = 0.05.

Results

The results showed that both individual characteristics and organizational factors had a significant influence on the performance of village midwives in ANC services, both in terms of quality and quantity. Among the variables under study, training and residence show a more dominant effect on midwife performance. This shows that increasing the knowledge and skills of village midwives, as well as the environmental factors in which they live, can contribute significantly to the effectiveness of the services they provide.

Discussion

The findings of this study have several implications for the improvement of village midwife performance. Firstly, the Babul Makmur District Health Center and the Southeast Aceh District Health Office are advised to encourage village midwives to follow further education, training, and seminars regularly to increase their knowledge and skills. Secondly, there is a need for policies that support the placement of village midwives who are experienced and have competence, especially from villages that require health services. Thirdly, supervision and guidance by the leadership of the Puskesmas need to be increased to ensure that the implementation of ANC is running according to the established procedures and standards.

Recommendations

Based on the findings of this study, several recommendations are submitted to improve the performance of village midwives:

Encourage further education and training: The Babul Makmur District Health Center and the Southeast Aceh District Health Office are advised to encourage village midwives to follow further education, training, and seminars regularly to increase their knowledge and skills.
Support the placement of experienced midwives: There is a need for policies that support the placement of village midwives who are experienced and have competence, especially from villages that require health services.
Increase supervision and guidance: Supervision and guidance by the leadership of the Puskesmas need to be increased to ensure that the implementation of ANC is running according to the established procedures and standards.
Balance professional and family responsibilities: For married village midwives, it is advisable to be able to manage the time between professional responsibility and family affairs in a balanced way.

Conclusion

The performance of village midwives in ANC services is influenced by both individual characteristics and organizational factors. Increasing the knowledge and skills of village midwives, as well as the environmental factors in which they live, can contribute significantly to the effectiveness of the services they provide. By implementing the recommendations outlined in this study, it is expected that the performance of village midwives in ANC services can be increased, leading to improved maternal and child health outcomes in the working area of the Babul Makmur District Health Center.

Limitations

This study has several limitations. Firstly, the sample size was relatively small, which may limit the generalizability of the findings. Secondly, the study only focused on village midwives in the work area of the Babul Makmur District Health Center, which may not be representative of village midwives in other areas. Finally, the study only investigated the influence of individual characteristics and organizational factors on the performance of village midwives, and did not explore other potential factors that may influence their performance.

Future Research Directions

Future research should aim to investigate the influence of other factors on the performance of village midwives, such as the availability of resources and the level of community support. Additionally, research should be conducted to explore the effectiveness of the recommendations outlined in this study in improving the performance of village midwives.
