Strategy for Increasing the Effectiveness of the Use of Antibiotics in Pneumonia Patients at the University of North Sumatra Hospital

Introduction

Pneumonia is a disease that causes the highest number of deaths worldwide, with its prevalence increasing every year. This has led to a high use of antibiotics, which unfortunately can trigger bacterial resistance and drug-related problems. The uncertainty in the use of antibiotics can have a negative impact on the results of treatment, the length of inpatient stay, and the patient's cost of illness. Therefore, this study aims to assess the effect of intervention on the rationality of the use of antibiotics, clinical results, DRPS (Drug-Related Problems), LOS (Length of Stay), and Coi (Cost of Illness) in pneumonia patients treated at the University of North Sumatra (USU) Hospital.

Background

Pneumonia is a disease that affects the lungs, causing inflammation and infection. It is a leading cause of death worldwide, with the World Health Organization (WHO) estimating that it causes over 1 million deaths annually. The use of antibiotics is a common treatment for pneumonia, but the overuse and misuse of antibiotics can lead to bacterial resistance, which can make the disease more difficult to treat. In addition, the use of antibiotics can also lead to drug-related problems, such as allergic reactions and interactions with other medications.

Methodology

This study uses a cohort analytic design with a retrospective approach for groups without interventions carried out from January to December 2018, and a prospective approach to intervention groups that take place from July to September 2019. The subject studied is pneumonia patients who do not have other infectious diseases and are 18 years or more treated at USU Hospital. The total research subjects consisted of 33 patients in groups without intervention and 42 patients in the intervention group. The rationality of the use of antibiotics is analyzed by the Gyssens method, while the DRPS identification is done using the PCNE V8.02 instrument. Clinical results are assessed from a decrease in the number of white blood cells (WBC), while LOS and Coi are measured to provide a clearer picture of the impact of this intervention.

Results

The results showed that there were significant differences in the use of rational antibiotics between groups without intervention (48.64%) and intervention groups (69.04%), with a value of P = 0.049. Although there is an increase in the use of rational antibiotics, analysis does not show a significant difference in the decline in the average WBC between the two groups, with the value of P = 0.950. In addition, there is no significant difference in the average LOS (p = 0.727) between groups without intervention (6.78 ± 2.43 days) and intervention groups (6.54 ± 3.29 days).

However, the results showed a significant difference in the incidence of DRPS, where groups without intervention experienced DRPS events of 72.7%, while the intervention group was only 45.2% (p = 0.001). Likewise in the average Coi, although there is a difference, the analysis shows the value of P = 0.061 which is not significant, with groups without intervention has an average cost of Rp. 4,994,200 ± Rp. 2,306,330, while the intervention group is Rp. 4,104,578 ± Rp. 1,739,132.

Discussion

From the results of this study, it can be concluded that the intervention carried out succeeded in increasing the rationality of the use of antibiotics in pneumonia patients, as well as reducing the incidence of DRPS that could endanger the patient. Although not all indicators show significant results, this research gives an important picture of the need for a better strategy in the use of antibiotics, to avoid resistance and drug-related problems. With the right approach, it is expected that the use of antibiotics can be more rational and contribute to improvement of clinical outcomes and reducing future maintenance costs.
