Introduction

Bacterial colonization is a significant phenomenon in the medical world, particularly in patients undergoing hemodialysis. In this study, we investigated bacterial colonization in the nasal cavity of terminal kidney failure patients who underwent regular hemodialysis, as well as staff at the Hemodialysis Installation H. Adam Malik Hospital Medan. Colonization is a process in which microorganisms such as bacteria are located and multiply on the surface of the body without causing infection. In general, bacteria that are colonized are normal flora found in the body, but can also originate from the hospital environment, which has a higher risk of causing infection in patients.

Background

Patients who are hospitalized, especially those who receive hemodialysis treatment, are at high risk of bacterial colonization due to sustainable exposure to microorganisms in the hospital environment. The longer the patient is treated, the more likely they are exposed to bacteria that can cause infection. Therefore, it is essential to understand the pattern of bacterial colonization, as well as antimicrobial sensitivity to these microorganisms. The hospital environment can be a breeding ground for bacteria, and patients who are immunocompromised, such as those undergoing hemodialysis, are more susceptible to infections.

Research Purposes

This study aims to determine the difference in the proportion of bacterial colonization and antimicrobial sensitivity to bacteria in the nasal cavity between regular hemodialysis patients and medical staff in the hemodialysis installation of H. Adam Malik Hospital. The primary objective of this study is to identify the types of bacteria that are commonly found in the nasal cavity of hemodialysis patients and staff, as well as their antimicrobial sensitivity. This information can be used to develop effective preventive measures to reduce the risk of bacterial colonization and infections in patients.

Method

This study is observational analytic with the latitude cutting method, carried out in the Human Rights Hospital Clinical Pathology Department in collaboration with hemodialysis installations from August to October 2016. The sample was taken through the anterior nasal nasal cavity swab, followed by the identification of bacteria and sensitivity tests on antibiotics. A total of 46 study participants, consisting of 23 regular hemodialysis patients and 23 staff, were included in this study.

Research Result

Of the total 46 study participants, the most commonly found bacterial colonization is as follows:

Staphylococcus epidermidis: 39.13% in patients and 52.17% on staff
Staphylococcus aureus: 17.39% in patients and 13.04% on staff
Staphylococcus saprophyticus: 8.70% in patients and staff

Of the total 23 bacteria isolated from hemodialysis patients, there are 3 MRSA (methicillin-resistant staphylococcus aureus) bacteria that indicate resistance to antibiotics, whereas in staff no MRSA is found. The presence of MRSA colonization in regular hemodialysis patients is a serious concern because it can increase the risk of more severe infections, especially in patients with weak immune systems.

Discussion

The results of this study are consistent with previous studies that have reported the presence of MRSA colonization in hemodialysis patients. A study by Gasemian et al. (2018) reported that 36.9% of cases of colonization of Staphylococcus aureus had MRSA levels reaching 74.2%. The presence of MRSA colonization in hemodialysis patients is a significant concern because it can increase the risk of more severe infections, especially in patients with weak immune systems.

Conclusion

From the results of this study, it was found that there was a colonization of MRSA in regular hemodialysis patients, with a level of sensitivity to Vancomycin by 50%. This shows the need for more effective preventive measures in managing the risk of bacterial colonization, especially MRSA, to improve patient safety during hospital treatment. Good handling and awareness of the presence of pathogenic bacteria in the hospital environment are very important to maintain the health of patients.
