Introduction

Type 2 diabetes mellitus is a degenerative disease that affects millions of people worldwide. Despite various efforts to find an effective treatment, the disease remains a significant public health concern. In Indonesia, the prevalence of type 2 diabetes mellitus is recorded between 1.5% to 2.3% in individuals aged over 15 years. In North Sumatra Province, the disease is ranked fifth in terms of outpatient visits in 2000. The high proportion of type 2 diabetes mellitus patients in Sibolga City prompted this study to design a type 2 diabetes mellitus control model through a risk factor approach.

Background

Type 2 diabetes mellitus is a complex disease that is influenced by multiple risk factors. These risk factors can be categorized into modifiable and non-modifiable factors. Modifiable risk factors include obesity, physical inactivity, and unhealthy diet, while non-modifiable risk factors include family history and age. In Sibolga City, the prevalence of type 2 diabetes mellitus is a significant concern, with reports indicating a 91.42% increase in the proportion of type 2 diabetes mellitus patients between 2002 to 2003.

Methodology

This study was conducted using an analytical approach with a case-control design. The research sample consisted of 100 patients with type 2 diabetes mellitus and 100 individuals without diabetes mellitus type 2, with similar characteristics such as gender, age, and shelter. Data analysis was carried out using a univariate approach, bivariate using chi-square, and multivariate through logistics regression.

Results

The analysis results showed a significant difference in the risk of type 2 diabetes mellitus based on several factors. The study found that there were significant risk differences related to family history, obesity, physical activity, and eating patterns.

Family History: Individuals with a family history of type 2 diabetes mellitus have an odds ratio (OR) 11.3, which shows a higher risk than those who do not have a family history.
Obesity: The obesity factor also contributes to an increase in the risk of type 2 diabetes mellitus, with OR of 4.9 for those who experience obesity.
Physical Activity: Physical activity is no less important, where individuals who have a bad level of physical activity have an OR of 5.2.
Eating Patterns: Eating patterns also affect risk, with OR 1.9 for those who have an unhealthy diet.

Conclusion

From the results of the study, it can be concluded that family history is the most dominant risk factor in increasing cases of type 2 diabetes mellitus in Sibolga. This study also succeeded in developing a mathematical model that can predict the fluctuations in the events of type 2 diabetes mellitus in the city. This model recommends intervention through cross-sector collaboration between the Health Office, the Education Office, the Indonesian Doctors Association (IDI), the Indonesian Midwife Association (IBI), and Persadia.

To overcome this problem effectively, it is important to implement a comprehensive approach that includes:

Wedding Counseling Program: Implementing a wedding counseling program to educate couples about the risks of type 2 diabetes mellitus and the importance of maintaining a healthy lifestyle.
Sports Activities: Encouraging sports activities every day to promote physical activity and reduce the risk of type 2 diabetes mellitus.
Balanced Diet: Maintaining a balanced diet that is rich in fiber to reduce the risk of type 2 diabetes mellitus.

Through this comprehensive approach, it is expected that the incidence of type 2 diabetes mellitus can be controlled and the prevalence rates can decrease in the city of Sibolga.
